New AHA Algorithm Encourages ED Pre-Activation and Direct-to-Cath-Lab Protocols

In emergency conditions like STEMI, patient outcomes depend heavily on the time between onset of symptoms to definitive treatment. As such, clinicians should make every effort possible to reduce any delays during that period. 

To that aim, an algorithm developed by the American Heart Association, and published earlier this week, establishes a standardized process which encourages pre-activation of the ED and determines when direct-to-cath-lab protocols are appropriate.

It's Easier to Pull a Rope Than to Push it.

My Grandfather (who was a very wise man) used to say “It’s a lot easier to pull a rope than push it.” Of course, as a kid I thought he had lost his mind. “Who would ever try to push a rope? It makes no sense!” 

I was right, it makes no sense. Yet, we find ourselves everyday trying to push ideas, initiatives, and process changes on others, and then we just can’t figure out why our “great ideas” don’t get off the ground.

We see this frequently in healthcare. My background is in ED Nursing and EMS, and in my roles in these fields, we pushed each other daily in efforts to get THEM to do things the way WE want them done.

The biggest pain point I encountered when working nights in the ED was communication — mainly, waiting. Call lists, pagers, and phone trees were our reality when we needed to act fast. But think about it: do we communicate this way in any other aspects of our lives? Why can I order a ride to the airport from a complete stranger with a few taps on my smartphone, but I need to use a pager to connect with providers I know personally? In medicine, when time is of the utmost importance, we still communicate like it’s 1985.

Technology and Data are Key to Transforming Stroke Care [TED Talk]

Dr. Brijesh Mehta is a Neurointerventional Surgeon, currently practicing at Memorial Neuroscience Center in Hollywood, FL. He recently gave a TED talk describing how his teams have leveraged new research, technology, and data to improve stroke care and give patients better outcomes. 

In the talk, Dr. Mehta describes how large vessel occlusion stroke (LVO) is "like a wildfire in the brain." He also says there used to be little we could do to stop the devastating effects, but now, thanks to new technologies and procedures such as thrombectomy, patients are regaining functionality and leaving the hospital much sooner than they had in the past.

Systems of care is a term that commonly refers to healthcare delivery that involves multiple organizations. EMS is deeply involved in systems of care for time-sensitive conditions like major trauma, STEMI, and cardiac arrest. Formal integration of EMS into those systems of care started in the 1960s and 70s – and the success of those early systems initiatives has continued to reverberate into this decade.

Didn't catch that. Oh well. Time to notify the rest of the team. Your unit clerk starts the process of trying to contact each member of the cardiac team. This takes her so long that before she's heard back from even half of them...
